1. Purpose of This Policy
Anagail Care Services provides specimen collection, diagnostic testing, and health screening services. This policy outlines how clients give consent for testing and how we handle their information safely and legally.
2. Client Consent
By scheduling or receiving any testing service, clients agree to:
- Allow Anagail Care Services to collect specimens (blood, urine, swab, or hair) for the requested test(s).
- Authorize release of specimen and test data to the authorized laboratory, medical review officer (if applicable), or requesting organization (such as employer or court).
- Confirm understanding of the test’s purpose, procedure, and any associated fees.
Clients have the right to ask questions before testing and may withdraw consent at any time prior to specimen collection.
3. Confidentiality
All client information and test results are kept confidential in compliance with HIPAA and other applicable privacy laws. Test results are released only to the authorized person or entity named at the time of consent.
4. Documentation
Clients will be provided with or sign:
- A Consent to Test form
- HIPAA Notice of Privacy Practices
- Any applicable Chain of Custody or authorization form (for legal, DOT, or employer testing)
5. Right to Refuse or Reschedule
Anagail Care Services reserves the right to refuse or reschedule a test if:
- Proper consent is not given,
- Identification cannot be verified, or
- Conditions may compromise specimen integrity or safety.
