At Anagail Care Services, our Diabetes Prevention & Education Program is a non-clinical, education-focused initiative designed to help individuals and communities better understand diabetes risk factors, lifestyle habits, and prevention strategies.

This program emphasizes education, self-awareness, and healthy lifestyle support — not medical treatment or disease management.

  • Blood Glucose
  • Hemoglobin A1C
  • Cholesterol screening (when applicable)
  • Body Mass Index (BMI)
  • Waist circumference

Fees

Diabetes Prevention & Lifestyle Support Session

$75 per session (60 minutes)
Includes education, lifestyle guidance, and goal-setting support.

Optional Screening Fees

Wellness screenings and CLIA-waived tests are priced separately if requested.

Mobile Service Fee (If In-Home or Onsite)

  • $69 per visit (includes up to 20 miles round-trip)
  • Additional mileage beyond 20 miles: $1.00 per mile (round trip)
